You do not need a sleep study for every sleep complaint. But when you do need one, it can be the turning point. The trick is matching the test to the question.
Quick mental model
Think of sleep testing as a diagnostic tool, not a routine checkbox.
- Home sleep apnea test (HSAT): focused tool for uncomplicated obstructive sleep apnea suspicion.
- In-lab polysomnography (PSG): full physiologic study when the case is more complex or broader data is needed.
When a sleep study is usually indicated
- Suspected obstructive, central, or complex sleep-disordered breathing
- PAP titration or re-titration when treatment response is unclear
- Narcolepsy workup (overnight PSG followed by MSLT)
- Suspected REM sleep behavior disorder or certain parasomnia/seizure differentials
- Persistent symptoms despite initial treatment
- Significant clinical change (for example major weight change with PAP issues)
When a study is often not needed first
- Straightforward chronic insomnia without red flags for another disorder
- Classic restless legs syndrome without competing concern for another diagnosis
- Circadian timing problems where history is clear
- Stable, well-controlled PAP users without new symptoms
Common mistakes
Mistake 1: "I cannot sleep, so I need a sleep study." Not always. Most insomnia is diagnosed clinically and treated behaviorally first.
Mistake 2: "A normal home test means nothing is wrong." Also not true. It may mean we need a different test or a broader differential.
Mistake 3: ordering the biggest test first when a focused test would answer the question faster.
Bottom line
Right test, right patient, right time. That is the entire game. A sleep study should clarify management, not just generate data.
