Stopping dopamine agonists such as pramipexole or ropinirole can cause a real withdrawal syndrome. Symptoms may be intense and sometimes prolonged, so taper plans should be deliberate and supervised.
What DAWS can look like
Common reports include anxiety, panic, depressed mood, agitation, fatigue, irritability, sweating, nausea, orthostatic symptoms, pain, insomnia, and strong urges to restart the medication. Presentation varies across patients.
Why this is hard
Resuming the dopamine agonist may reduce withdrawal symptoms, but it can also reintroduce the problems that led to stopping, including augmentation, impulse-control symptoms, and sleep attacks. That tradeoff is why planning matters.
Who may be at higher risk
Risk appears higher with longer exposure and higher doses, especially in Parkinson disease cohorts. DAWS has also been described in restless legs syndrome at lower doses. We still do not have a single proven taper protocol.
Practical steps
- Do not stop dopamine agonists abruptly unless your clinician tells you to.
- Build a gradual taper and follow-up plan with your prescribing team.
- Discuss alternatives early (often alpha-2-delta ligands for RLS).
- Track mood, sleep, autonomic symptoms, and function during transition.
Bottom line
DAWS is clinically important and can be prolonged in some people. If you are transitioning off a dopamine agonist, use a supervised taper and close follow-up rather than a sudden stop.
